It should include, but not limited to the following: </p> <p> </p> <p> a) Purpose and Objectives </p> <p> b) Background Information </p> <p> c) Steps/Guidelines (how to start, selection of a model to use such as the plan, do, act and check model (PDCA) or six sigma.) </p> <p> d) Useful Measurements/Evaluation Tools (such as Affinity diagram, Causal loop diagram, Cause-and-effect diagram (or fishbone diagram or Ishikawa diagram). </p> <p> e) Implementation Process </p> <p> f) Example of an authentic project (examples to use: improving the emergency wait times at a hospital or quality of healthcare at the primary care physicians office) utilizing the </p> <p> steps/guidelines you have provided </p> <p> f) References (at least three different sources – including your textbook materials) </p> <p> </p> <p> Students should be creative in developing this document, which should answer fundamental questions that a novice would ask about quality improvement projects, such as: </p> <p> 1) What do I need? / What should I include? </p> <p> 2) Where do I start? / What should the focus be? </p> <p> 3) Why is it important? </p> <p> 4) Are there any guidelines for doing this? </p> <p> 5) How do I determine what needs to be improved? </p> <p> 6) What tools do I use to collect and analyze data? </p> <p> 7) Whom do I involve during the process? </p> <p> 8) How do I implement the improvement? </p> <p> </p> <p> </p> <p> </p> <p> Guidebook needs to be at least 8-10n <a name="_GoBack"> </a> pages; however, refer to the rubric regarding how students will be graded.
